DHFWS Birbhum Recruitment 2024 How to Apply
Eligible candidates have to appear before the ‘District Level Selection Committee’ of the District Health and Family Welfare Society, Rampurhat Health District located at MNK Road, Old Outdoor Campus, Kamarpotty Mor, Rampurhat, District Birbhum, Pin- 731224, W.B. on the said date i.e. 06th March 2024 at 10.30 AM along with the following documents in original and one set of self-attested photocopies –
i) Application as per given proforma
ii) Madhyamik exam admit card as proof of age
iii) All Marksheets of MBBS Degree, PG Diploma/ PG Degree, etc. with the mentioned degree and diploma certificate.
iv) Registration certificate from West Bengal Medical Council.
v) Certificate of experience working as a Medical Officer in any Govt. /Semi-Government/Private Hospital/Nursing Home, if any. An appointment letter will not be treated as an experience certificate. Volunteering will not count as work experience.
vi) Voter Card/Driving License/Passport/Aadhaar Card as proof of identity.
vii) Breed proof certificate, if applicable
